• Vision Screening (Kindergarten and 3rd grade)
• Dental Screening (Kindergarten and 9th grade)
• TDAP (7th grade)
• Meningitis (7th and 12th grade)

All 7th-12th grade athletes will need an up-to-date physical/concussion/medical information sheet.
