Health Requirements
- All students need a current immunization record
- Vision Screening (Kindergarten and 3rd grade)
- Dental Screening (Kindergarten and 9th grade)
- TDAP (7th grade)
- Meningitis (7th and 12th grade)
Sports Forms
All 7th-12th grade athletes will need an up-to-date physical/concussion/medical information sheet.
